Every kill of an ordinary monster has a chance to spawn a stronger version of it, marked with a real skull above its head. The more dangerous the skull, the bigger the reward. It works across the entire map, so any spawn can surprise you.
Every kill of an eligible monster has a 3.5% chance to spawn a skulled monster.
The monster does not appear instantly. For 5 seconds the tile pulses with the skull's effect, and the deadlier the skull, the faster it pulses.
A living skulled monster pulses its effect every few seconds, so you can spot it in a crowd of ordinary monsters.

Killing a black skull always leaves an extra reward in the corpse: 1–2 exalted cores, on top of the multiplied loot.
Killed a red or black skull? There is a 15% chance that a Pale Crystal awakens where it died. It works across the whole map, not only in the Hazard Zone.
It has 20 times the health of the monster it grew from. A pushover on minotaurs, a proper fight on demons.
Every 8 seconds it summons two copies of the same monster, up to 6 at a time.

A skulled monster counts toward tasks and charms just like an ordinary one. The server also keeps separate counters of skulled kills.
One player can trigger up to 10 skulled monsters per 5 minutes (up to 7 of the same type), so you cannot mass-farm them off a single monster.
A group with shared experience active shares the limit, which grows with the number of players up to a hard cap of 30.
A skulled monster never spawns another skulled monster. Summons, bosses and monsters without experience never roll a skull.
